Hi. I have just received all my medical notes. I have to now send. Is it better to send the report listing everything, every appointment? I have missed many hospital appointments due to anxiety. I was shocked at how many. I have investigations that have not been finalized by my cardiologist until the MRI report is done. Also waiting for further appointments. So should I mention this? My spine condition goes back years and my surgeries, so should I include as far back as 2001?
My doctor told capita on there form I was suffering depression, neuropathy, and chronic back pain.
I wondered if you can advise also who I can ask to come with me as I am isolated.
I have asked for the tribunal office to send my bundle twice now over the phone and they still have not sent.
can i send more evidence as I get it. I really have to send it tomorrow.

If the panel has asked for your medical records then send everything to the Tribunal Service.

Yes, you can send new evidence but the panel can only consider your conditions and limitations as they were on the date of Decision being appealed.

For someone to attend with you. Realistically none of the advice agencies will provide assistance this late in the process. You may be able to find someone from a local voluntary group, your GP Surgery should have a list, also, try any charities that specialise in your conditions.

Remind the TS that if you do not have a copy of the papers then your hearing is likely to be adjourned again.

Thank you for your reply. i called DWP again requesting my bundle for the third time so hopefully, it will arrive sometime soon. So you are saying send the whole disk even though there are many other things that do not apply? Sorry to keep asking the same question.

It is not for you to decide what is relevant and what is not in your medical records, I agree, most of it will be redundant but that is for the Tribunal panel to decide.

Are you calling the DWP for the bundle? If you are then you should not be, it is the Tribunal Service who should be supplying a replacement.
