esa50 form and medical was created by jimmy.royle
hello, my name is jimmy, I have been a member 3 years, I have learned a lot from reading lots of messages on forums, however I think this is the first time a have written on this board. I have to fill an esa50 form and then probably go to the dreaded medical. A lot has changed for me in the last few years, I have had 3 tuis, a stroke, encephalitis and have been placed in a induced coma, as well as an osteotomy to my right let, which was not successful, which has left me in severe pain. I have a weakened right side of my body, I also have a loss of memory, I was wondering if I could take my partner, she is fab, she knows all dates of hospital dates, names of consultants, and lots of other things. just not sure if I could take her to my medical. thank you so much for listening. Jimmy.

Hi Jimmy

You are allowed to take someone with you to a medical. If you have memory problems make sure that you clearly show this on your form. When you go to the assessment, you need to remind the assessor at the beginning of that issue and that you may need your partner to speak for you if you forget. Now you have to be careful here because the assessor will want to hear from you first, but the assessor should establish how much they will allow your partner to talk. (It does vary from one assessor to another).
