If some one prepares my food an average of 4 days a week do I tick yes or sometimes?

I would say yes because it is for the majority of the time, but you can also leave the boxes blank and write 'see below' and then write your exclamation in the box.

Just to add to the advice already given, it is not enough that someone prepares your meals for you four days a week, you need to explain why you cannot reliably do so on those days. You will be tested on what you cannot do not what you do not do.
