Pip review form was created by Miss B C Dawson

Struggling to complete the pip review form. Not sure whether to complete as no change as ongoing pip was won by appeal in 2017 so obviously original pip application did not meet criteria. Any advice as to how to complete based on this and what supporting documents to include? Do you include appeal outcome? Please advise
