Really appreciate your comments. I will contact the DWP about obtaining the complete assessors report - the decision maker quoted so much of the assessor in their decision letter that i have probably wrongly assumed that it was the whole report. Curiously the senior assessor was also present throughout my assessment and sometimes asked questions of her own. Could she have made notes/ a report too?
My CMH despite trying its damndest to sabotage all PiP claims, has a part time worker whose devotes 50% of his time to writing PiP reports from patient medical records. Because i refused a voluntary section and narrowly escaped a compulsory section (they had a court order), the CMH has point blank refused to write a report on me. When i went to ask about a freedom of information request they had their biggest burliest orderly silently menace me by standing directly in front of me.
My Gp surgery has suddenly become very vague and evasive - they seem to be taking their lead from the CMH - but you are right, i will and must WRITE to them. Not that i expect much of a result. When i learnt that the sole Therapy Centre in my area had completely banned me (without even seeing me) i put in 2 Sars to find out why and how. The first was lost and the second has been outstanding for 2 months now.
I did tentatively explore the possibility of a MH advocate with my 3 local mental health charities to help me challenge the sabotage arrangement but they claimed that they could not help. And being on the NHS blacklist (locally and nationally) is a huge hindrance in getting any help/ being treated fairly.

If you are being denied access to your medical records for the reasons you said in your post, then you certainly need to include this information in your MR, because they need to understand not only does it not allow you to give a fair account of your medical history, which they apparently are not taking seriously.
I would consider making a complaint to your local CMH and ask why it appears you are being denied help when they are offering it to other people.

I didn’t know that this report existed until I was making a call regarding my mandatory reconsideration. It constituted of 30 pages of absolute rubbish. The way in which the phrase in word things is always within the positive. A simple example would be that I required a tissue because I became distressed as I took the tissue from the box myself this meant I had a good grip and pinch ability! On receipt of this report we went through it and wrote a five page response of all of the errors which it contained. None of this was taking into consideration in any shape or form. Know any of the medical evidence it was a complete waste of time.

Sorry you are all alone with this, its NOT your fault you are ill.
If you want to make a complaint about your treatment or lack of it by MH services you are eligible for a specialist MH advocate (Independent Health Complaints Advocate)
0300 330 5454. If face to face meetings tend to get out of hand, stick to emails. There might be MIND professional advocacy in your area:sadly none in mine.
Your advocate would have access to your records & should be able to unblock access to help with a PIP appeal.
I have witnessed people being denied food, clean clothes,visitors & books/pastimes to force
compliance to treatment in MH hospital so refusal to help with money to live on is in the same abusive human rights violation.
I have also experienced discharging & dumping patients that they are not competent to treat & blaming the patient!
Maybe a good advocate can move things along for you. Sounds like you need a MUCH better psychiatrist.
I found the MIND legal line helpful. I try to write down what I want to say/what help I would like before I phone otherwise it all gets bogged down & frustrating. You come over as pretty bright so I'm sure you are aware of Rethink Mental Illness helplines?
Some people on the Forum have actually paid for a private psychiatric report for PIP. I use my PIP for private psychotherapy:its a different world.

Hi, I hope this will be some comfort, if cold comfort to you. I do know of a claimant who, though their mental condition is different to yours, has had similar experiences of "lost" "untraceable" etc medical and claim records and documents. Only because they are very systematic and persistent and keep records of their own of everything they DO get their hands on, and send everything special delivery regardless of expense, have they now won multiple times over the years and got lifetime awards. But this has come at the cost of several 'safety valve' suicide attempts, fortunately unsuccessful, out of sheer frustration, along the way, and I would say permanently shot nerves. They suspect that they are persecuted for being too good at 'beating' the DWP to get what their severe mental and physical impairments deserve.
