hi, I have had a permanent stoma because of colitis, this was done last march, but I still suffer from colitis as its now in my back passage. my question is ,do I say iv colitis on the esa form?i would be grateful for any help. thank you in advance. emily
You need to list all your conditions you suffer from and from when they started, you also need to say how each condition affects you, the same goes with any medication you are taking for your condition(s), does the medication have any side effects if so what effects do they have on your daily life, do they make you lethargic for instance.
So in answer to your question, yes state that you suffer from colitis and that you have a permanent stoma and what affect it has on you now.
