Hi,
Having completed my PIP claim form due to my current form ending I wondered if anyone can advise on a statement I have written for the "extra information" section before I submit it to see if there is anything obvious that will make it more difficult to get an award without the need for tribunal again.
I think I have answered the questions well enough and have a statement form my physio saying that I am unable to walk more than 20 meters and also a care plan from the local Councils Occupation Health Assessors. The council have also renewed my Blue Badge on the report from their assessors.
If you feel able to assist me please message me as I am unsure about putting the statement in my post for all to see.

It is very difficult for us to comment on what members might write in their forms because we have no understanding of your conditions or how they might limit your ability to complete the PIP activities. At best we might be able to point what is wrong but we can't tell what is right and more importantly what is missing.

Hi Gordon.
Thanks for the response. I would be happy if you could point out what is wrong as this is what I was hoping for.
The statement is below which I am hoping it clarifies that I receive assistance daily and that my issues are affecting my abilities across the questions and not specific to just one question if that makes sense.
"I am now at the point where driving a vehicle is becoming too much for me and taxis are being used for simple short journeys more frequently though these pose challenges also. I am now also realising that a mobility scooter will help me outside of work and I am looking into purchasing one as my condition is getting worse as I become older.
The way I move around also has an effect on the way I can perform the tasks below:
• washing & bathing
• dressing & undressing
• managing toilet needs
• preparing food
• eating & drinking
• mixing with other people
• managing treatments
• going out
I receive a lot of assistance at home from my family on a daily basis and without this assistance I would find it hard to manage by myself and the thought of being on my own is worrying. I need help to carry objects, as even a small weight on my arms has a negative effect on my pain levels. My wife is my main aid and will do anything to help me daily but she is unable to help me when I am at work and for this I have colleagues that will assist me though they do not know fully the difficulties I have at work especially with my personal needs.
I f I could get more help at work with things of a personal nature then this may help the way I feel or if it was possible for my wife to come to work to assist me when I needed help with the toilet I would manage better however I am unable to discuss these issues with my employer for fear of ridicule or deciding to end my employment. I don’t know what I would do if I had to stop working but the thought of this is becoming more real. Work gives me a reason to get out of bed and the satisfaction I get from working helps me mentally and enables me to focus on something besides my issues though every day is difficult."

What would be useful to go with your claim is a supporting document written by your wife and or anyone else who gives you support on a regular basis.

Would it be possible to comment if there is anything in the statement that shouldn’t be written that may hinder the chance of being given PIP for any of the questions on the claim form?

It's a final comment by you so it is really down to you to decide what you include but some general comments.
The issues you raise are not directly connected to the PIP activities, there is nothing wrong with this but you will only get an award by meeting the Descriptors and scoring the points.
You mention your mobility has an impact on your completing a number of the activities have you explained in detail in your response to each of them why this is the case.
You mention that you work, have you explained how you manage this whilst still being limited in completing the activities.
