Dear Gordon,

I received a letter instructing me to not do anything during the lockdown period but to wait to hear from them.
I am wondering if they are able to do an assessment without the form or if something has gone wrong?
Is it usual to conduct a telephone assessment without a claim form?
Is it possible a new form has gone astray in the post, or should I requesting one now?

I am so unsure what to do now.

Whether to send the doctors letter requesting a paper-based review to the assessment service and DWP even without my full ill health and disabilities listed?

Or contact DWP on Monday to request a paper based assessment and asking what my next steps are regarding a paper based review?

I am wondering whether sending evidence in dribs and drabs or not including my full ill health and disabilities may disadvantage me if they decide to assess me without a form, if that is a possibility.

I was wondering what your thoughts or advice would be?

I'm really sorry but your lastest post has confused me more.

The first post in this topic says you have received a letter to arrange a telephone assessment?

So, I've already provided answers to your questions if you look back through this topic.

The decision on whether to do a Paper assessment lies with the Assessment Provider first, the DWP can override this decision but not until your file is returned, so you need to contact the AP.

Dear Gordon,

Apologies if I have confused you,communication is difficult and problematic for me.

I have received a letter to arrange a telephone assessment.
However, I have not completed a form.

The AP said that the only exceptions to telephone assessments for paper based assessments are for terminally ill individuals.
Reading through the forum it appears this is incorrect.

There is no legal requirement for a form to be issued for a reassessment if I had to guess then yours has been missed.

The person who told you that it is only the terminally ill is incorrect, a Paper Based review can be done when the assessor has sufficient information for one to be produced or where there is evidence that carrying out a telephone or face to face assessment would cause significant ditress to the claimant.
