Hello,
Ive been on ESA (support group) since its inception and I am due for reassessment soon.
Will I be reassessed using the new ESA forms or original ESA form?

Both forms are the same apart from the name ESA50 / UC50.

You should be assessed using the ESA50 as this is a legacy benefit.
