hello all. need advise i sent my uc50 form on the 18th sept 2020 so had not heard anything from chdauk {health assessment advisory service spoke to them today and they told me they had done report on a paper based assessement cause of the medical records i had sent with the uc50 form and they had sent the completed form back to universal credit office on the 24th sept 2020.i phoned universal credit today and they told me they have no records of form being sent back to them, what shall and can do now knowing the chdauk confirmed today to me on phone they had sent the form off to them

Hi diceman24

Poor you. I would contact the health assessment service again and see if you can get to talk to a senior person. Either they need to send the report again - or they need to write you a letter confirming that they already sent it - so you have something to say to the UC office. If you get no joy from them - go back to the UC - say that you need to speak to a senior person, explain the dilemma and ask what can be done. This is clearly not your fault and they need to take responsibility for sorting it out.
If you still don't get any joy - contact your MP.

thank you for quick reply when i spoke to u/credit on 6th april 2021 the lady has written on my journal she will chase the case manager up and she also wrote it should be added to my next payment date if i have lgwra
