PIP renewal completed by carer was created by Jane 21
Hi hope all are well. I like so many need to complete paperwork for the person I care for as their conditions make it difficult/impossible for them to do so. My question is how do I word it that I helped to complete the form so that they are aware that the person cared for cannot as per all the info in the PIP. Don't want it twisted. Thank you. Hello Jane, and well done for stepping up to help the person you are caring for!
I would suggest that you are completely up front about it. Complete the form in the third person - 'XXX is unable to prepare food for herself because ...' In the additional information section you can include a statement from yourself which says that you completed the form for XXX because... Assuming the person has the capacity to do so, I also include a statement which says that I have either read what I have written to the person, or they have read it, and they agree with content.

Should be a box on the form where it asks if someone has completed the form for the claimant. Ensure you fill it as failure to do so could be problematic for the claim.

Thank you for the advice. I assume "completed the form" means the one who is physically having to fill it out for the claimant even if the claimant is having some input when they can, ultimately it is the one physically writing up on their behalf. Therefore the 3rd person. Apologies. Stressing.

Hi Jane
Yes the one physically filling out the form, follow Catherine's post below and explain why a third party is completing the form, if you regularly receive help from this person, ask them if they will write a supporting letter as to what help they give and why.

Thank you. All gets really complicated, we all wish we could have normal lives. Thank you again. We have worked over months on her diary at the times when she was most coherent so it is in her voice but I typed it up as she cannot see or physically function to do this. Should I put any extra wording on this incase they say "she must be ok then"?
