Hi I hope you can help I sent my ESA form in back in March last year last October I asked if they are looking into it and said they yes and last week asked again about it on my Journal on my universal credit now they said they don’t know nothing about it am have Learning difficulties I cannot remember which address I sent it to because my support worker did it for me I wonder if you can give me the address where the ESA form goes to I live in Congleton Cheshire am waiting for my support worker to visit me in May to help me so if if could tell me the Address would be a big help I think it’s Wolverhampton but not too sure thanks
We can't tell you which office your paperwork is sent to - because there are a number of different offices. However, I have included the Wolverhampton office address.
