I write as I need advice. I have received an unexpected and unrequested Permitted Work (PW1 04/20) form. There is no covering letter and the correspondence is undated. I am most concerned about how I should respond.

A close reading of the boiler plate on the form indicates that it must be filled in if I wish to do any work. Having read the form there is no space to say I am not doing work. That is, the PW1 assumes that if you fill it in you are doing work.

I have not been doing any work. I am highly disabled, with lung problems in addition to other issues. In fact I was eventually issued with a shielding letter and essentially urged into protective isolation. Even attempting to do any work would be deeply dangerous.

I can see three options. The first is to do nothing. I have no request to fill in and return this form. By its plain text I should complete it if I wish to do Permitted Work. The second is to fill it in but endorse it making it plain I am not doing any work. The third is to write a letter explaining that I was shielding and have not nor will not be performing any work as it is simply too dangerous.

What are people's opinions about this?
