That is a confusing one. If it had not all happened so quickly I would have suggested that someone had made a paper based assessment and sent it to the DWP for a decision. But will it all happening so quickly I think your assumption could well be right, that someone has seen the previous PA4 and logged it incorrectly as the current one.
It is hard to know how to how to advise you. If for some reason they decide to base his new award on the previous report would you be happy about that, if so, you might want to do nothing. If however you want to make sure that everything is done by the book and possibly avoid delays then I would suggest that you ring them for clarity.
