DLA 434 FORM - DLA - CHANGE CIRCUMSTANCES

Is there a guide for filling out a DLA 434 - Disability living allowance ,looking again at a claim for a person age 16 or over/ child under 16?

I really need to find some guidance on applying to gain higher rate care component and mobility component.

Any help much appreicated?
