  			 				Apologies if this is a repeat question but, i'm struggling to find an answer.
 
Background:
I currently get PIP at the enhanced rate for both.
I have both physical and mental health comorbidities
My partner gets Carers Allowence for me.
Previously we got Child Tax Credit and Income Support as a couple.
I've never claimed ESA (nor am I eligible for new style ESA)
We started managed migration to UC at the end of September 24.
I returned my completed UC50 form around Xmas 24.
 
Last Tuesday, an Assessor from Independent Assessment Services (0800 260 0700) randomly called out of the blue.
I was asleep and my partner panicked and just handed me the phone.
My anxiety kicked in (it also have concentration issues due to meds ect). I was very groggy & don'think I made much sense, going off on tangents .
 
I didn't have the time to record the call; which lasted 7 mins and 3 seconds
She seemed nice, but I did not hold out much hope.
 
On Friday evening there was a message / letter - I have been placed in the LCWRA group.
 
Now with PIP I've always requested the assessors report (as advised by these forums - thank you again).
 
So my questions are:
 
1. Was that a "Paper" assessment?
2. Which form do i request - UC85 Or UC85a?
3. Do i need to request anything else?
4. I'm planning on requesting this via the phoneline (is this the best way)

You say you cannot remember exactly what was said but I think you have had a paper assessment and you can phone and get this confirmed and a copy of the report which is called a UC85a. I believe you have to request it by phone as Journal messages go to either the Work Coach at the Jobcentre or the Case Manager at the UC Service Centre who don't have access to it.
