what form to ask for post pip assessment was created by freddey
hi, had my pip phone assessment today. what form do i ask for to see what the accessor`s opinion is and when shall i phone up for it.

Hi freddey

The form is called a PA4 (Assessors Report) you can ask for a copy of the form 48hrs after your assessment.

We are told by some members that they receive a message from DWP informing that they have received the assessors report, if you receive this message then contact DWP for a copy.
