I'm sorry to hear about your PIP. If you asked them to record the assessment, you can request a copy. They don't automatically record them.
I can't tell you whether you should put in for a Mandatory Reconsideration - that's something you will have to decide for yourself, but you have already said what's in the report doesn't match what you said. It sounds hard to go from enhanced mobility to 4 points.
As for the daily living score - you already have enhanced (but didn't score as highly as you were hoping or expecting. Some people just accept the points that they have been given (mainly because they don't want to rock the boat, or are just too exhausted to keep fighting) and others insist on challenging the score even when they have been awarded enhanced (usually as a matter of principle when they feel they have been undermarked or worry about the impact on the next review). The only right decision is that one that feels right for you.
