Firstly, read pages 134 - 138 of the Guide to PIP Claims and Reviews. This explains the review process and the AR2 review form.
In those four pages it will tell you to look back in the guide to earlier pages in order to answer each of the questions.
You can find the pages you will need from the contents on pages 2 and 3. So if you are answering a question about being able to cook a basic meal (Preparing food) you will find the advice on page 33.
