Help confused trying to do PIP application for Daughter

So daughter is 18, ASD, Anxiety and depression, PCOS, B12 deficient, folate deficient, vit D deicient , had major surgery for severe Scoliosis 3/4 years ago . She asked me to apply for her , I did the PIP test on here says she could get enhanced on both, of course we know that's not a given. I'm really struggling to get it done, thought I'd go through the copy of PIP test on here to get started but the questions on the form from them is bit different from the last lot I did and the one on here it's thrown me. It's due back to them on 29th, already had a 2 week extension, could I get another? Just having a wobble
