Condition worsens / changes between IB and ESA mig was created by Doris
If your current IB award is primary for mental health issues and in later years you develop more physical medical conditions. Are you supposed to notify the DWP of a change in condition - even if it means you've got worse ?
If you don't, what happens when you're transferred over to ESA and add these new conditions onto the form ?
( Noting on your IB letter, it only states the total points you were awarded, not the proportion between mental health and physical descriptor. So you've know way of knowing which way its weighted )

As far as I know you are required to inform the DWP if you have a change in circumstances that has a materially affect your entitlement to benefit, so it's most important when you condition improves.
When you are transferred to ESA you should state any extra conditions and detail any deterioration in condition.
I can remember many years ago I was told that I scored at least 10 points for mental health or 15 points for physical health, which at the time didn't mean much to me, but I would certainly recommend that everybody obtains a copy of their medical report following an award to find out what points they have.

As of January 2011, all IB assessments ceased, so there will be no requirement to refer to a previous medical report for a PCA examination.
The DWP will be looking at previous medical reports when transfering IB claimants to ESA, but I don't believe that a 5 year old report would be considered relevant, because of its age and the differences between the ESA tests and the IB ones.
