Hi,the same thing has happened to me,i received mine last week and my name and nat ins. number are only on the front page,also my report is almost illegible.I am going along the lines of writing in my submission for the appeal that i dont know if the report refers to me definitely as my name and nat ins number are not on all the pages and asking if they will disregard the report because of this and it being illegible.Hope you have some luck with your appeal.
