A bit confused

Hi am awaiting pyschological therapy at my local cmht and am on a waiting list, where would I put this on the questionniare as I am awaiting treatment. Thanks

Hi P,

You could mention it on page 5 of the ESA50, About you Cont, Hospital or Clinic treatments, outpatient or inpatient.

You would have to state that as of this time you do not have a date, but are on the waiting list.

Or simply mention it on a seperate A4 page.
