DBD370(n)

Hi, i am currently completed yet another DLA form as the current award is due to run out. i am desperate to be able to supply as much supporting evidence as possible. Do you know if it is possible to get the forms required to be completed by health professionals involved in the care of a claimant? I know that the DWP doesnt like to ask for extra info and i wanted to ensure that i get it from GP and Mental health team involved in care.
