Esa 50 form question.

Esa 50 form question. was created by Noodles
Just about to send off my downloaded B&W version of the ESA50 form . I then noticed on the last page of the original smaller esa 50 form sent last week that it says I should send back the form THEY sent me or words to that effect ?

Hi N,
It is perfectly acceptable to send the PDF ESA50 that you downloaded back to ATOS.
You do not have to the send the pink one back that you received through the post.
You can keep that one as a souvenir, you may find some use for it though, maybe in the bathroom ???
