W received the esa50 today to migrate from IB to ESA. I have two quick questions (I'm sure they have been asked before). I will be filling in the form for my daughter as I am her primary carer. Which is fine but I am having a long over due holiday at the end of September for three weeks. I have people (friends etc) to take care of my daughter while I am away. Is it a problem to put a three week period in which we cannot attend a medical? She is not on holiday you understand I am. However the people I have taking care of her will not be able to help with a medical. The other question is do I have to put a telephone number? We have NEVER received a call from ATOS only ever had letters. Which is much better for my daughter to deal with. She cannot use the phone.

There should be no problem with you placing restrictions on when your daughter can attend the assessment, however, be aware that ATOS often attempt to book the appointment without sight of the ESA50, and therefore it may be necessary for you/her to move it to a convenient date, given the timeframes, this could be an issue that you will need consider how to deal with.
You are not required to put a phone number on the form, but it is not unusual for the DWP to have acquired a number from other sources, so you should include a statement on the ESA50 that she only wishes to be contacted in writing.

So if ATOS send us a date for a medical assessment and I am on holiday will my daughter be able to get someone to call up and change the date for after I return? She is starting to stress about it now. She really does NOT want to attend the medical without me there. I live with her I know and see exactly what she can and can't do.
