IB to ESA transfer do I need a P45 or sick note ?

I'm presently in the process of preparing my ESA form to send in.

I'm being transferred from IB to ESA.

Do I need to send in a P45 and sick note with it ?

Or is it like I think, that these things are only needed for someone making a new ESA claim after just leaving work ?
