The ESA and DLA regulations refer to severe discomfort, there is no formal definition of what constitutes severe discomfort, however, case law suggests that it is less than severe pain.
So yes, you can refuse to carry out something that would cause you severe discomfort, you should explain why you don't want to carry out the action, and specifically refer to the fact that it would cause you more than significant discomfort.
