Can anyone enlighten me as to what a Med 3 form is? I recall Med 4 forms from years ago when I first claimed but have never seen or provided a Med 3 form since. i wonder why it's been referred to on the ESA85A form and what relevance it is?

The MED 3 "fit note" replaced the old "sick note", see :
www.cpag.org.uk/content/fit We have heard of the MED 3 being referred to in member's ESA85A's on many occasions. Even though the members are positive that their G.P. never issued a MED 3.
We believe this is because the ESA85A contains phrases that are electronically generated by the LiMA software used by assessing ATOS HCP's.
It seems that in these cases, the HCP has not bothered to amend the phrase, even though a MED 3 is not available.
