Hi Dq,
It is supposed to be sent to claimants several weeks before their CB ESA in The WRAG is due to run out due to the 365 day entitlement limit, to assess them financially to see whether they are going to be entitled to IR ESA when their CB ESA ceases.
Some CB ESA recipients in The SG, may be asked to fill one in to assess whether they are entitled to an IR ESA top up to their CB ESA, as one can receive both CB ESA and IR ESA.
