IB - ESA notification was created by Julie
Should notification be received regarding change from IB to ESA before an ESA 50 form is sent for completion ?

I see there is a form IBM350 but I never received this stating what the conditions for conversion are.

I received an ESA 50 which I completed and returned with a letter from my GP and then received an appointment to attend a medical assessment.

I have supporting medical documentation from my main consultant regarding exceptional circumstances.

Do I send this information prior to the assessment or take it with me along with other supporting information ?

Yes, you should have received written notification of the transfer and been offered the opportunity to ask any questions you had on it, before receiving the ESA50.

With regard this additional evidence, I would take a copy with you to the assessment and ask the ATOS assessor to read it before they start the interview and then include it in your file when it is returned to the DWP. This is covered under section 3.1.2 of the WCA Handbook.

I would also send a copy to your local DWP office. marked for the attention of the Decision Maker, to ensure that it does not get lost.

Good luck at the assessment.
