As I said in my previous post, if you do not have written confirmation that you do not need to complete this form, you should assume that you do.

the person on the phone said i was best still filling the form in and sending it back and i would not hear from them again,!!!!!

the form i have is the ESA50 01/13 (bottom right hand corner of the form)
is this form downloadable and which guide is best to follow?
