I've tried in vain to find the ESA3 form which I need to complete along with the ESA1. Can you help?

As far as I am aware the ESA1 captures all of the information that is requested on an ESA3, so it would be worth your contacting the DWP to confirm that it is required, however, if it is then your only options are to request one from the DWP or to see whether you local JC+ office can supply you with one. There is no electronic copy of the form available in the public domain that I am aware of.
