I am once again finding myself completing an ESA50 form for my husband. Seeing that they only gave him a 3 month diagnosis for the support group.
My question is as the form relates to firstly the physical functions, then the mental, conitive and intellectual functions - should I still list all the problems he has completing the tasks listing in each question or only cover the problems relating to the topics i.e Part one - Physical, then Part Two - mental, etc.
I hope I explained that OK
He is already in recent of DLA high MOB and HIGH care and I dont wish to hinder this benefit as well.

DWP advise that they can consider only problems caused by a physical condition in the first section and problems caused by a psychological/mental problem in the second part. However our recommendation, as always, is that you make the form work for you and put things into whichever descriptor/s that you believe are appropriate.
