information from DWP was created by CE2025
I was placed in support group of ESA recently but the letter does not state for how long.
I want t ouse your template letter to send to the DWP to try and find out this information.
Do i just send it to the address on the letter they sent me informing me i'm in support group?
Also what is the LT54 i need to request?

Congratulations on getting the correct decision!

If you just want to know the 'prognosis' you could phone DWP and ask whether it is on the computer screen.

If you wish to submit a Subject Access Request you should send it to the address of the office that has been dealing with your claim. Mark SAR in the top left-hand area of the envelope.

Thank you, Can anyone tell me what the LT54 form is i need to request please?
Also i think i need a copy of the ESA85A form too

The LT54 is a DWP form completed by a Decision Maker, it is used to document revisions and transfers from another benefit to ESA.

The ESA85A is the medical report produced by ATOS from information in your ESA50 and any evidence attached to it. The prognosis is usually used to set the re-assessment date.
