Forgot to mention assessment recording!

Hi,
Just posted my ESA50 form as my renewal is due, but have forgot to
request on the form that if called for an assessment I wish to have my medical
recorded!

Do I send a separate letter to ATOS now requesting my assessment be
recorded if called for one or do I wait for them to contact me for an
assessment then request?

Hi B,

I would send it now to the same address as your ESA50 was returned to.

Put your name, NI Number, and DOB on any pages that may be included.

You could also include a covering letter explaining that you forgot to include it with the ESA50.

If you do receive an appointment for a face to face, I would resend it then to ATOS, by post and e-mail.

Requesting written confirmation that they have received your request, and that they are going to comply.
