Hello

I have applied for DLA and my application was received by them on Friday the 19th July, which was the date on my claim pack.
I received a text message yesterday acknowledging my claim for DLA. Only I've just discovered looking at my paperwork that I have forgotten to send of my supporting evidence with the claim pack.

I would like to know if its too late to send it in and if not how do I send it in ?

The other thing is I've been unable to obtain recent supporting evidence as my GP doesn't do this and my NHS consultant will charge around £200. Also at the hospital the other people involved like the physiotherapist say that the DWP will write to them and they will only provide it the. To the DWP.

So the evidence I have is a number of years old that I had from private consultants I saw. I had X-rays and MRI scans with them and have severe OA in both hips and need 2 total hip replacements, OA in both knees, OA in lumbar spine and degenerate disc disease. Will they a accept this as evidence ? or is it pointless sending it to them ?

Hi dumpling,

It seems that an increasing number of GPs are refusing to provide evidence directly to the patient. In these cases it might be worth asking to view your medical records and taking copies of anything that might be useful

However, for now you will have to do the best you can with what you have got. Send your evidence to the same place that you sent your form to. Mark the envelope 'For the attention of the Decision Maker' and enclose a note asking for this to be taken into account along with your form. Make sure that your name, DOB and NINo are clearly marked at the top of each page of evidence.
