I've been getting info from the site for some time but recently joined as the dreaded brown envelope arrived

Your guides are fantastic as the questions and descriptors are a minefield

I have a query though ... apart from my gp and family I don't have any support workers etc to write on my behalf, is there anything you can suggest? Also is it best my husband fills the form, or me with him

Fortunately or unfortunately I am attending some of my hospital consultants in the next 3 weeks, should I get letters from them too?

You can have a look through your medical records for supporting evidence
Requesting your medical records
Charging for medical records
If you are unable to physically complete the form yourself then it is OK for your husband to do so on your behalf. If he is using your words then he can complete it as "I" and you can sign it, if he is using his own words the he should use "she" and he should also sign the form.
If your Consultants are willing to provide information then you should

Back to the stupid form ... it gives me a headache and only manage one question a day before done for due to my fibromyalgia
