HELP!!! how to fill in ESA Forms

Can anybody out there give me some tips on how I am supposed to answer the ludicrous questions on the ESA form?? Am really struggling.

WOW the ESA50 form tool ages to complete. I had to wait for my partner to help me to do it, we were both mentally drained after 2 hrs.
I do not know who would have ever devised such a non distinct questioner, it took me several reads to actually attempt to understand what they wanted from me. It was only with the help of my partner and your guide that I was finally able to complete this form.
