Hi,
starting to fill my SSCS1 form and have a few questions, if anyone could help.
1. Sections 3 and 4. My husband is my main carer. All DWP correcpondence is addressed to him ("Dear Mr.X. I understand you're acting on behalf of Mrs.X. The information in this letter is about her). We had a DWP person coming to our house, and my hubby signed something to confirm that all official correspondence will be going thru him). Does it mean that he (quote from SSCS1) has been formally appointed to act on my behalf by DWP and should fill in Section 3 with his details (or is it with my details? How do we fill in Section 2 then?)
2. We don't have have any additional evidence to send to the tribunal. However, how reliably will DWP send all medical evidence, seeing how economical with the truth they were during my assessment? Shall we rather send the evidence again ourselves? Do they "forget to send" or "lose" evidence?
3. My condition is variable and I may or may not be able to attend the tribunal (my GP will probably agree to confirm my inability by a letter to the tribunal). However, my husband is my main carer and has first-hand knowledge about my day to day life. Shall we have smaller chances of success, if my husband attends tribunal without me? In this situation, shall we still tick "I want to attend hearing of my appeal." in Section 6? We don't want paper hearing.

1. From your post, your husband appears to be your DWP appointee, if this is the case then they should be completing and submitting this SSCS1.
So your details go in sections 2 and 3 and your husbands go in section 4.
2. The DWP will as part of their submission provide a copy of all the documents used in the making of the Decision, this should include your claim form and anything you attached to it.
At this stage, you should not need to send any documents that you have already sent as part of your claim. When you receive the DWP's submission you can check to see if there are any documents missing and can then supply them yourself.
3. It should not be a problem if you cannot attend but I recommend, once the appeal is accepted, that you write to the Tribunal Service asking permission for your husband, as your appointee, to attend on his own.
