Hi there,I was just wondering if you have have any idea which box i should tick for the questions on washng and dressing ,it gives options for both needing help with upper an lower body now I need help with both so do I tick both boxes of do I tick cannot do it at all ?? Do you have any links for a subject access request for g.p records, I have scoured the internet and can find a template letter sorry if this is not appropriate question .

You don't have to use the tick boxes if they do not represent the problems you with this activity but you do need to answer them in the comments box.

Can you wash yourself with the use of a suitable aid of appliance?

Can you wash yourself with assistance from someone else?

Remember you need to complete each activity reliably and on the majority of days so factor this in to your replies.
