Hi I have just received how your disability affects you forms. I have some letters from my hospital and attended a pain management programme but not sure what to send, Very nervous about filling in the forms. Any advice please. DABA

If you think that the reports are supportive then send them, the most important thing is to explain why they are relevant to your limitations in completing the PIP activities.

Make sure you use our PIP Claim guide to help you complete the form and remember you need to be able to complete each activity reliably and on the majority of days.
