On Saturday, i recieved a letter and form from Capita, for my partners pip claim, although it referred to me as DR.
I am not a doctor, and we did not state i was his doctor. and this distressed my partner, as he felt like they are not reading the forms correctly, anyways, i telephoned Capita today, and they said they have also sent a copy of this form to my partners actual doctor, and one to his mental health nurse for them to fill in and send back to them, They said i would need to phone the DWP, which i did, she said it states that i am named as a health or social care professional on the form. ?
I was reading back through the forms we sent back and in the original claim form (pip1) form, as we had to do ask for a paper form for my partner
assets.publishing.service.gov.uk/governm.../pip1-claim-form.pdf On page 12, it asks who the main health care professional that supports you we filled in his doctors info, then the 2nd question is further health or social care that supports you. :-
Please tell us if there is another health care or professional or someone else we can contact who can tell us about your illness. For example a community Psychiatric Nurse, Occupational therapist , support worker, Social worker,
carer, or family member or friend.
And for this question we filled in my name , stated i was partner/carer
I stated to the DWP about this and she said she couldnt give me an answer as she didnt know what the form was so i would have to call Capita, ended up going round in circles, i said to the DWP that i would just fill out the form, as i was probably the best one to actually comment on his day to day routine etc being as though i was with him 24/7, and then they had the option to choose or not choose to use this . And she agreed.
His doctor and the mental health nurse, although they have not seen him in a long time, as he has not left the house in 2 years, know that he is applying for PIP, and have already sent letters of support, so hopefully they will have enough info to fill these forms in.
