I have received an invitation to attend a F2F medical assessment, I called the DWP and asked why have I received this appointment when I have not been invited to complete the ESA50 questionaire . I was told that I had completed one last year and it was found that I needed to be seen by a Health professional... Now this is the bit that will cause confusion I have never filled one in so how can there be one filled in ?. should I ask for a copy to be sent to me to verify it was me and how would II go about resolving this matter please.in the meantime I have arranged for my medical appointment to be moved to another date. also I have new medical evidence and should I take this to the F2F or send it to the DWP. Thanks in advance

Just to make sure that you are aware, having moved your appointment you must now attend the new appointment or your file will be returned to the DWP who will likely close your claim.

It is perfectly possible that there is no ESA50, unfortunately, the ESA call centre staff are known to make assumptions about things like this without substantiating whether it is true or not. You can request a copy of this form from the DWP office dealing with your claim.

You can complete an ESA50 and take it to the assessment with you, you can download the form from the following

You can take your evidence to the assessment, if you do ask the assessor to read it and include it in your file before they start the interview but I would also send it and the ESA50 if you do one to the DWP office dealing with your claim.
