hi all thanks for your comments,can anyone advise on my problem of not having any upto date medical evidence,i know if i asked my G.P to send me to a consultant,he would just say no point because there's nothing that can be done which he has told me many times when i have seen him.also is it me thats thick but i can't seem to put anything together in the dla guides to explain your typical day,you can't say you watch t.v all day,you cant say you move round the house or garden(weather permitting)you can't say you go supermarket,i am finding it all rather confusing.my day comprises of watching tv,then i will go and sit in conservatory and have a read,most days because of pain and discomfort,and dizyness i go back to bed for a few hours because i dont sleep well at all.would this be acceptable i don't know.what are members views.thanks

Hi Libra

I agree, a Review sent out under The Right Payment scheme is a slightly different concept to a Renewal or New Claim. They are fairly infrequently sent.

The form is DLA80 and you have 3 weeks usually in which to return it.

Apologies if I referred to the wrong situation, however the information in the B&W guides I posted will equally be of assistance, in the case of a Review.

Hi P26,

The best advice that I could give, is to state it as it is.

If you have to live a very limited lifestyle on a daily basis, then state that.

Work on a typical day say, from midnight to midnight, and go into detail, as to how your needs/disabilities/limitations have to be taken into account in all your day to day living.

It can be difficult, as one becomes used to living within one's capabilities.

Try making a list of every normal daily action, and how carrying out said actions may be affected.

Don't forget, if you need help and care, when carrying out activities, you do not have to be receiving said help and care, as long as it is "Reasonably Required" it counts.

Don't assume that the person assessing your DLA award knows anything, state even the simplest fact, and don't be afraid of repeating yourself.

As for medical evidence you can obtain full copy of your G.P. records, by requesting said copy at the reception.

You could then include any relevant extracts. There will be a charge that can vary from one G.P. practice to another.

It is possible that your G.P. will be asked to fill in a report by the DWP, see information on such reports here :

Even though your G.P. may be aware of your medical condition, make sure he is also aware of the resulting limitations, and any care needs.

It may be prudent to make an appointment with your G.P. to discuss this.

bro58 thanks for your reply and the useful links about medical records,but as you can see i only have 2 weeks to get form back so would not have time to get copies of records,unless i phone them and ask for more time,but i would not be to hopeful for this to be granted.
