Need Advice about medical evidence

I've filled out my ESA50 using your guidelines as I am being migrated from IB PCA exempt to ESA. My GP has given me a strong letter of support where she states that I have 2 to 4 weekly infective exacerbations of asthma ( Brittle asthma). I''ve stated this also in the form but am unsure whether to get a print out of my acute prescriptions as evidence as they will show the frequency of antibiotics and steroids prescribed. I did ask a receptionist last week for this but she printed out the last months acutes. Would it be beneficial to ask again and use it as evidence.
Thank you

Providing supportive evidence of what you have stated in the ESA50 is important so yes, I would include this.
If you have not yet returned the form then simply attach the additional evidence.
If you have returned it, then I would send a copy with a short covering letter to the ATOS address you sent the ESA50 and the DWP office dealing with your transfer, if you do not know this, then just send it to the office that usually deals with your IB.
Sending Documents to the DWP or ATOS
If you send the information, then I would also take a copy to the assessment, if you have one and ask the assessor to read it and add it to your file.

Will the letter of support from the in which she states the frequency of exacerbations and Discharge summaries from the last two years showing infective exacerbations along with a respiratory nurse letter mentioning frequent exacerbations be enough. I'm getting a bit worried now.
Thanks

I would get a copy of the last one that was issued and send that in with your letters.
