You should be able to request a copy of the report, it is normally referred to as GP Factual Report, from the DWP office that dealt with your claim, however, you are normally required to lodge an appeal with one month of the Decision letter, so I would be careful to do so within this timescale even if you have not received the report, as the appeal can be rejected if it is late.
