My condition means that some of the tick boxes on the form don't apply to me, is it ok to leave them un ticked with a see below for details, then explain in writing.

Hi T,

Yes it is fine for you not to tick any of the boxes that you feel do not apply to your own limitations, with a "see below" comments and details provided in the text box below, or on a separate referred to A4.

We always advise that members should not be constrained by the tick box options provided within the ESA50 if they feel that they do not pertain to them personally.
