I have been on esa /ib for some years just over 12 months ago i had a medical with the compulsary forms to fill in . a few days ago i recieved a medical letter for february but no forms to complete etc .

i hate these medicals ,they look at you as though your lying through your back teeth . but the thing that is worrying me is i thought there were no reviews for 2 years and why just a face to face assesment with no form to complete.

last year was a horrenduos year for me and family ,my my suffered cancer for the 2nd time which makes everyone miserable , i then started having absence seizures lots of them , anytime any where . i then suffered a grandmal fit in bed at home ,woke in hospital in agony . was discharged so hours later as i was already been seen by an epilepsy specialist . while in the foyer of the hospital i suffered yet another grandmal . the discharged me within 24 hours so that i could go out of one door and back into A+E for a spinal scan to find i had slipped 2 disks .

within a month or 2 i then suffered a massive heart attack brought on by nothing .im only 42 not over weight dont drink etc.
took 3 hours for them to confirm a heart attack due to incompetence (suing) ended up with stents . i am now on 15 different medications daily some 4 times a day the others twice .

i had a needle in the spine as they could not operate due to 40% muscle loss in my heart due to the time it took to do angioplasty. i was supposed to be having microdiscoptomy .
it did relieve the pain for a while but it is back and i am now on codiene 4 times aday plus diazipam 4 times a day just for the pain.

my life now consists of confusion worry and pain. depression also rears its head if left to think about it .

thought 2015 would be better , my absence siezures have calmed down ,only to be replaced by 2+ hour episodes of what can be descibed as unconciousness
seeing a new consultant (higher up ) he describes them as coma states .
i have been told not to be left alone ,go out alone ,or bath alone .
then i get this assesment letter.

then this week my son 14 years old ends up in hospital after 3 weeks of constant sickness to be told he may have stomach cancer.

letter states to bring in evidence , i have some letters but only 1 about the heart attack . which is a letter of admission of gross failure of the paramedic/ambulace staff .

how can i obtain a medical report to hand in that has all these things i have wrong in one letter , as my medical reports are like the old yellow pages, thick and by the time a few pages are read youre bored.

Although the DWP announced that there was a two year deferment of re-assessments, this has never been 100% as we have had members receiving ESA50s through out this period.

I would visit your GP immediately to see whether they can help you or not in regard to some kind of report or letter, if they say that ATOS will right to them if necessary, you should point out that you already have an assessment booked so the point when ATOS will write to the GP has long past.

You can contact the DWP and ask them to delay the assessment so you will have time to complete the ESA50, you need to speak to a manager, the call centre staff do not have the authority to defer the appointment, it's a long shot but it may give you more time, you can also contact ATOS and ask them, but doing so may count as one of your two tries to attend, so definitely go with the DWP first.

my wife phoned atos today and asked them why a ftf medical without an esa50 form ,the man replied that because of back logs they are not doing the esa50 form and are just doing ftf to update.

my wife explained that i had previously had one in october 2013 and filled in a esa50 beforehand .

he just replied with bring in any medical evidence you have .

i still have a pdf backup from my previous esa50 form ,that could be updated with all the new info , but i havent been requested to hand one in . do you think it wise to update it ,print it out and take it with me .

as for a doctors letter my regular doctor moved away to canada only a few months ago so my new doctor (but it can be any as there are upto 3 week delays to see a regular doctor) has no real knowledge of my health only what is on his computer screen . the last time i went to see him he had to open a book to look up what some of the tablets actuallly did .

The ESA50 is the only opportunity that the claimant has to state their limitations in regard to the ESA Descriptors so if you can produce one in advance of the interview, then I would certainly recommend that you do.

If you have time send it to ATOS in advance of the meeting then you should, otherwise take it to the appointment and ask the assessor to read it before starting the interview. I would also send a copy to the DWP office dealing with your claim, marked for the attention of the Decision Maker.

my wife has just had a hard time trying to explain the situation every person she spoke to just said "oh you dont need one " she put phone down and re-dailled more than once until she finally got someone with some intelligence who said yes you should have one sent out beforehand .

so i am going to just get her to update the old one with all the new medical info , whats changed in my health condition and ,physical ,mental restrictions plus all medication adjusments and add ons .

then send it recorded delivery tomorrow directly to atos
