I have not seen this letter before and I am not aware of anyone else on this forum or any of the other forums that I monitor having received one.
It is extremely unusual for the DWP to send mail with a hand written envelope, I am not even sure that they can.
All letters from the DWP should include your National Insurance Number.
The only contact number that the DWP would use is 03456 088545.
Based on what you have posted, I am concerned that this letter was not from the DWP, you may be able to check its status by contacting the DWP on the above number and asking them to confirm that they have sent it.

thank you very much for your reply, I've been worried about this but when I rang the number which is 0161 257 1244 they said they were job centre plus so I don't know if this makes a difference or not.
I have also spoken to other people who I know to be on esa and they haven't seen one of these either but one person said they had and that they will be investigating me about something as it happened to them and 4 days later they received another letter, this time with the names and addresses on and were told someone was going to visit them.
whoever I spoke to said they are sending them to everyone but it's strange know one else has had one except this one person.
I really do appreciate any information given, thank you.

thank you for your reply, I am worried about this myself but the number that was given was not 0345 or anything like it, it is just a normal number, which I rang. the woman said they were job centre plus and that they are sending them out to everyone but know one else has had one except one person I spoke to who said they had one and 4 days later they got another one saying they were being investigated and that they were going to visit them, which is what happened so I am now thinking I am being investigated for something or other.

As Gordon has stated, this letter is not the norm.
I am also most surprised that you have been given a direct phone contact number for The JCP, trying to get these direct contact numbers is generally like trying to get "blood out of a stone"
All DWP contact numbers given are generally to a Call Centre as with :
THE ESA PHONE NUMBER DWP : 0345 608 8545 Press Option 3 for ESA.
I would do as Gordon has advised and ring the number above.

I was just wondering if you are in the 0161 area code JC+ catchment area because not that number certainly had trouble with scam telemarketing companies working from that area code so please be careful with any details they may try to glean from you
